CLINIC-BASED TELEHEALTH ROLE FOR EXPERIENCED APP - SURPRISE, AZ

We are seeking an experienced Family Nurse Practitioner (FNP) to join our primary care team, focused on delivering value-based care through a virtual telehealth platform . We are looking for someone comfortable with telehealth driven care that will also support fellow clinical team members thru high quality, team based virtual care.
Some Details About the Position:

• Clinic based onsite in Surprise, AZ
• Flexible scheduling: 4x10s or 5x8s shifts
• Medical Assistants, Front Office, and leadership support
• Support of an integrated system including specialists, urgent care, and more!Essential Functions and Requirements:
  
  • Provide comprehensive, high-quality care and support to established patients via tele visits
  • A portion of time will be of support of inbox management for your team members
  • DEA with prescriptive authority required
  • FNP designation with AZ license eligibility
  • Minimum (1) year of clinical experience as an FNP in a primary care setting
